What Insurance Do You Accept?
We are enrolled with Blue Cross Blue Shield, UnitedHealth, Aetna, Medicare, and Medicaid. We’re always adding new insurance, so call us if there’s one you’d like us to consider.
Many of our patients are self-pay due to our package called, The Signature Care Sessions- Your time, your health—our only focus. Choose 20 or 40 minutes of uninterrupted, personalized care, where they enjoy concierge-level care without the concierge-level price. Prices are $150 and $190 respectively.
I have a High Deductible. Can I use my HSP Card?
Definitely. Because we offer high quality 20 and 40 minute visits for only $150 and $190 respectively, many of our patients are self-pay or use their HSP card.
What About Refiils?
We’re not like most doctors office- we’re trying to help you to optimize your health and take a few medications as required. We’re constantly working to optimize your health, and must track our progress in your improvement. This requires a team approach, complete with regular visits and monitoring.
Refills are provided during these office visits and not between appointments via the medical assistants or receptionists.
If necessary, your pharmacist can request a 30-day refill, which will only be approved after a follow-up appointment has been scheduled. Running low on your medication serves as a reminder that you are likely due for an office visit. Dr. Butler prescribes a sufficient supply to last until your next scheduled appointment.
Schedule Your Follow Up Labs & Appointment at least 14 days BEFORE you run out of medications. The purpose of each appointment is to assess your medical condition and determine whether a medication adjustment or discontinuation is needed, particularly if you are on our, “Get Off The Meds” program.
How much medication can I get at the time of my appointment?
If you have no issues and are stable on your medication, you can usually receive a 90-day supply with one refill (for a total of 6 months). Birth control medications may be refilled for up to one year, as they are considered preventative rather than for treating a condition.
For ongoing but stable medical issues, a 3- to 6-month supply may often be prescribed (e.g., high blood pressure medication). However, if your condition is not well-controlled or requires close monitoring, you will typically need to return every 1-3 months for medication adjustments and evaluation (e.g., diabetes). Patients with dangerously uncontrolled conditions may require follow-ups more frequently.
The key takeaway: prioritize getting your health issues under control and ensure you request refills during your office visits. This will prevent situations where your pharmacist informs you, “Sorry, you’ve run out of medication.
Why can't I just call when I'm on my last pill like at other doctor's offices?
Of late, there has been an epidemic of medical/medication errors that negatively affect patients. The average office is overwhelmed with refill requests, often requiring additional staff to manage the high volume. This contributes to frequent medical mistakes.
Approximately 30% of phone calls to the average office are related to refill requests, and another 20% are follow-up calls from the same patients who did not receive a timely response.
For example: “It’s 4:45, and nobody called me back! I just took my last pill! Help!”
To ensure efficiency and fairness for all, please schedule your follow-up appointment 2 weeks BEFORE your medication is due to run out.
What if I'm in a pinch- are there exceptions?
Pharmacists will usually give you a 3-5 day supply to tide you over in an emergency. We make exceptions for situations in which you have had an emergency or are out of town.
I am sick. What about calling in antibiotics?
Dr. Butler does not prescribe medications, including antibiotics, based solely on a phone call. Patients need to be seen and evaluated to ensure accurate diagnosis and proper treatment. While an occasional urinary tract infection (UTI) or yeast infection may be treated without an office visit, recurrent cases—more than two UTIs or yeast infections—require further evaluation and possibly a referral to a specialist, such as a kidney doctor.
Prescribing medications without an in-person evaluation can lead to missed diagnoses. For example, what seems like a UTI could be a kidney stone, a suspected upper respiratory infection could be asthma, or a sour stomach might actually indicate appendicitis. To provide safe and effective care, in-person visits are essential for most conditions.

For form completion, patients must schedule a dedicated appointment to allow adequate time for the provider to review and complete the form. Patients are required to complete their portion of the form BEFORE the appointment, as blank forms will not be accepted.
A $75 fee applies for forms that are submitted for completion outside of a scheduled appointment or presented after a medical visit, as a “drop off.”
Examples of forms subject to this fee include, but are not limited to:
- Disability forms
- Return-to-work forms
- FMLA (Family and Medical Leave Act) forms
- School, camp, or work physicals
- Handicap Permits
- Illness/Injury
- Insurance Applications
- Foster/Adoption Applications
- Prior Authorizations for brand name drugs
- Authorizations for Out of Plan Health Care
- Letters written on your behalf for school, lawyer, judge, employer, or health and well-being verifications.
